Personnel and Training Required

An interviewer who is trained to conduct personal interviews with individuals from the general population is required. The interviewer must be trained and found to be competent (i.e., tested by an expert) at the completion of personal interviews.* The interviewer should be trained to prompt respondents further if a "dont know" response is provided.

*There are multiple modes to administer this question (e.g., paper-and-pencil and computer-assisted interviews).

Diagnostic Criteria:

One or both of two subcategories can be present for diagnosis of rheumatic fever (RF) or rheumatic heart disease (RHD), according to the World Health Organization (WHO) Criteria:

(a) RF: Two major or one major and two minor manifestations plus evidence of a preceding group A streptococcal infection.

  • Major criteria are carditis, polyarthritis, chorea, erythema marginatum, and subcutaneous nodules.
  • Minor manifestations include fever and polyarthralgia; EKG-prolonged PR interval; elevated laboratory evidence of inflammation (erythrocyte sedimentation rate, white blood cell count, C-reactive protein).

(b) Chronic valve lesions of RHD: presentation with pure mitral stenosis or mixed mitral valve disease with or without aortic valve disease. Do not require other criteria for Rheumatic Heart Disease diagnosis.
