Description
This protocol includes 10 interviewer-administered questions from the Reproductive Health section of the National Health and Nutrition Examination Survey (NHANES) 2013-2014 that capture use of female hormones (including pills and patches) and duration of use.
Specific Instructions
The Sickle Cell Disease Genitourinary Working Group notes some questions in this assessment are sensitive. Investigators should provide a private space for completing assessments and to be prepared to offer appropriate support.

Personnel and Training Required
No specific training is needed if data are collected through a self-administered questionnaire. If interviewers administer the questionnaire, the interviewer must be trained to conduct personal interviews with individuals from the general population and found competent to administer these particular questions (i.e., tested by an expert) at the completion of this training. The interviewer should be trained to prompt respondents further if a “don’t know” response is provided.
Equipment Needs
The PhenX Sickle Cell Disease Genitourinary Working Group acknowledges these questions can be administered in a computerized or noncomputerized format (i.e., paper-and-pencil instrument). Computer software is necessary to develop computer-assisted instruments. The interviewer will require a laptop computer/handheld computer to administer a computer-assisted questionnaire.

Compared to infertility questions found in other instruments, the National Health and Nutrition Examination Survey (NHANES) questions were selected because they excluded questions about contraception and collects detailed information about the type(s) of estrogen used.

Definition
Hormone therapy refers to the use of medications and patches that contain female hormones.
Purpose
Estrogen and progesterone use (used independently or in combination) has been linked to cancer risk. Assessing use of these hormones will help quantify their association with either increased or decreased risk for cancer. Additionally, in female patients with sickle cell disease, the hormone(s) used alter the individual’s risk for blood clots and stroke, and so understanding which medication(s) have been used throughout the lifetime is important to understanding that individual’s risk for certain complications.
