Description:

This is a two-item measure of an individual’s weight change goals used in the 2015 Youth Risk Behavior Survey (YRBS).

Specific Instructions:

Although this was originally used with grades 7-12, the PhenX Working Group members recognize that it is important for a broader age range of adolescents and adults, and a couple of states have included the question in their BRFSS.

Protocol:

1. Which of the following are you trying to do about your weight?

[ ] A. Lose weight

[ ] B. Gain weight

[ ] C. Stay the same weight

[ ] D. I am not trying to do anything about my weight

2. How do you describe your weight?

A. Very underweight

B. Slightly underweight

C. About the right weight

D. Slightly overweight

E. Very overweight
