This protocol includes parent-report questions about how the COVID-19 outbreak affected the child and family members to date. 

Specific Instructions

Another protocol may be used for children age 13 and older to self-report effects of COVID-19 on the child’s life.

1. Did the child’s school/preschool/daycare close because of the COVID-19 outbreak?
